- All Postgraduate and Honors Students are to fill in THIS FORM for the request to publish their survey advertisements on the MUPA Website.
- All Honors and Higher Degree by Research (HDR) students must provide their project title and ethics code and a letter of consent from their Course Management/Research Office for which the survey is to be published.
- All Coursework students must provide their Unit Code and a letter of consent from their Course Management Office for which the survey is to be published.
- The postgraduate student must provide a brief summary of the objective of the survey which is to be published online and the necessary contact details.
- A JPEG/PDF version of the survey advertisement must be provided with accurate details.
- The survey advertisement would be uploaded on the MUPA Website for a maximum duration of 2 months for which the student can request for an extension to retain their survey advertisement on the website for up to a maximum of an additional 2 months.
- The approval from the MUPA Editor and the MUPA President (or his/her proxy) would be required before the survey advertisement can be published on the MUPA Website.
Note: The process of approval would take at maximum 10 working days from the point the request is made.

- All Honors and Higher Degree by Research (HDR) students must provide their project title and ethics code and a letter of consent from their Course Management/Research Office for which the survey is to be published.
- All Coursework students must provide their Unit Code and a letter of consent from their Course Management Office for which the survey is to be published.
- The postgraduate student must provide a brief summary of the objective of the survey which is to be published online and the necessary contact details.
- The approval from the MUPA Publicity Executives and the MUPA President (or his/her proxy) would be required before the survey advertisement can be published on the MUPA Website.
- All areas where advertisements/posters are to be placed must be declared by the postgraduate student.
- Once approved, the postgraduate student must provide the print outs of their survey advertisements and/or posters for approval by the MUPA Publicity Executives. Print outs which are NOT stamped are unauthorized to be placed on noticeboards.
- The maximum duration of a survey advertisement posted is for a period of 90 days for which the student can request for an extension to retain their survey advertisement on the noticeboards for up to a maximum of an additional 90 days.
- A deposit of RM20 would be imposed and would be refunded once all survey advertisements and/or posters are removed from the noticeboards on the date of expiry. All advertisements must be presented to MUPA Publicity Executive to receive their deposits.
- Deposits would NOT be refunded if,
- the applicant did not place their survey advertisements and/or posters at restricted areas and/or areas which have not been declared to the MUPA Publicity Executives.
- the applicant did not remove his/her survey advertisements and/or posters from the noticeboards once it has reached the expiry date.
- the applicant places additional survey advertisements and/or posters which have not been stamped and approved by MUPA Publicity Executives.
- the applicant places their survey advertisements and/or posters on the walls, doors, lifts and any other unauthorized areas within Monash University Malaysia.
